Aflac Voluntary Benefits
While major medical pays the doctors, hospitals and labs, Aflac pays money directly to you. Aflac benefits will help with co-pays, deductibles, mortgages, rent, car payments, and food for you and your family. INCONEN has secured group rates and the premiums for some plans may be payroll deducted on a pre-tax basis. Choose from Accident, Cancer, Specified Health Event, Disability and Life based on your own individual needs.

Blue Cross / Health Insurance Plan
Knowing that a typical hospital stay is now more than $32,000, we
have selected 43 excellent medical plans from Blue Cross. These individual and family plans include 40 PPOs and 3
HMOs. Also available are the new consumer driven Health Savings Account compatible plans. Blue Cross offers a broad range of health plans that vary in
price and health coverage levels so that you can choose the plan
that's right for you. Blue Cross is so flexible you can even choose
a different health plan for each member of your family. Please
remember that the Blue Cross provider network is the largest in the
state.*
Blue Cross / Vision
As a member of Blue Cross, you will be eligible for the Eye Med Vision Care program for eye exams and glasses. You are also eligible for TruVision for Conacts and Lasik. *
Blue Cross / Dental Coverage
For as low as $11 per month for a single individual, you can
obtain dental insurance. With Blue Cross there are 3 Dental HMO
plans and 4 PPO plans. Again, there is a range of dental plans that
vary in price and benefits.*

DHS/Dental Coverage
Enroll yourself for only $21.20 per month, yourself and 1
dependent for $39.60 and yourself, and 2 or more dependents for only
$57.10 .
INCONEN offers group rates for a dental coverage with Dental Health
Services. Though many of he services do require co-payment
participation, several services such as X-rays, examinations
cleanings, etc. are fully covered. No claim forms, no deductibles
and no limit on the amount of provided services.
IRC Section 125
If you choose one of the Aflac, Blue Cross or DHS plans, INCONEN can set
up the Premium payments to be payroll deducted on a pre-tax basis.
Automatic Depositing of Paychecks
You do not have to worry about getting to the bank on time. No
fees. Will commence approximately three (3) weeks after enrollment.
Paid Holidays
You may be paid for the following holidays: New Year’s Day, Memorial Day, Independence Day, Labor Day,
Thanksgiving and Christmas Day. Holiday pay for part-time employees will be prorated if eligible. Check with your
recruiter to determine if you are eligible for this benefit. This benefit is based on the contract signed with the end user. |
